Medical Errors

Today’s health care system is complicated and hard to navigate — just when you think you’ve got it figured out, things change. Whether you’re a patient, doctor or pharmacist, chances are you’re facing or may face a host of issues. Prevent Medical Error (PME) is a non-profit organization dedicated to identifying and in turn, reducing medical errors and factors that set the stage for them, such as:

  • Medication Errors such as an allergic reaction
  • Questions about generic drugs over brand name drugs
  • Drug shortages and recalls
  • Health care cost increases and confusion about what is covered that cause delays and disruptions in care
  • Patient safety gaps such as inaccurate or incomplete diagnosis
  • Insurance disputes and therapy or drug formulary authorizations and restrictions
  • Transfers of care as pharmacists as ObamaCare takes effect and physicians merge, sell and close their practices
  • EMR – Electronic Medical Records – glitches and other computer glitches
  • Increased paperwork and red tape
  • Fragmented care and inability to focus
